Alliance of Chicago is leading the Smart Health Center project, which places trained health information specialists in low-income clinics to assist patients in connecting to their own medical records and find reliable information about their own conditions.
This initiative is funded by BTOP PCC money for computers & equipment along with a grant from the Otho S. A. Sprague Memorial Institute for training and patient engagement around technology.
The project includes extensive documentation of process and procedures so that all interested parties can learn from our work.
The program is currently being deployed in two health centers in Chicago. More details to come.
